PHOTOGRAPH/VIDEO RELEASE FORM

I, the undersigned, hereby grant Real Matters Inc. and/or its affiliates (including, without limitation, Solidifi Corp. and Solidifi U.S. Inc.) (collectively, the “Company”) permission to photograph and/or video my likeness and to use such photograph(s) and/or video(s) in any and all of their publications, including, but not limited to, printed and digital publications. I understand and agree that all photographs and/or videos of my likeness will become property of the Company and will not be returned to me. I further consent to the use of my name by the Company in connection with such photographs and/or videos.

I hereby irrevocably authorize the Company to edit, alter, copy, exhibit, publish, or distribute such photographs and/or videos for promotional purposes or any other related lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears.

I understand and agree that I will not receive, and hereby waive my right to receive, any payment, royalty or other compensation in respect of: (i) the publication of such photographs and/or videos of my likeness; (ii) the use of my name in connection with such photographs and/or videos; and (iii) my time and expenses related to taking such photographs and/or videos.

I hereby hold harmless, release, and forever discharge Company from all claims, demands, and causes of action, which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I acknowledge and confirm that: (i) I am at least 18 years of age; (ii) I have read this release and fully understand its content and meaning; and (iii) all questions pertaining to this release have been answered to my satisfaction.
